Primary Health Care Dietician - Supermarket Tours

Description
A community dietetic service is provided to whanau and community groups. Venues include health centres, marae, community halls and homes. The Primary Health Care Dietician provides tours in several supermarkets in Whangarei. Through the Primary Health Care Dietitan service, Manaia Health PHO provides dietetic and nutrition expertise, resource and support to several community projects. These include gardens, health kai inititatives and educational workshops.
The supermarket tours focus on reading food lables and are an integral part of patient's diabetes Self Management Plan.
The target population includes:
1) Individuals referred by the GP who are in need of specialist dietary support relating to chronic health conditions
2) groups of predominantly Maori clients identified by two Maori Health Providers/partners of Manaia Health PHO.
What Worked Well
There were 136 new patients referred from GP Practices in 2009-2010 year. 40% new referrals Maori/Pacifc. 5% new referrals Asian/other ethnicities. 36 Referrals diabetes related. 8% bowel irritation related. 24% referrals CVD related; 13% referrals nutrition related. This lead to 440 interactions with clients.
