Programme: 8 Week programme with weekly contacts. After the first two face to face meetings fortnightly phone contact is made. Focus is on dietary changes patients are required to complete a diet record and this information is entered into the Dietplan computer software and together we the community worker form Te Puna Dietary goals are set. Other holistic support is provided as indicated e.g. budgeting or parenting support.
