Hunter and New England Diabetes Alliance: innovative and integrated diabetes care delivery in general practice

This study evaluates a quality improvement programme for diabetes healthcare, the Diabetes Alliance program, aimed at coordinating services for rural isolated clients. The integrated care programme was rolled out to over 80 practices and was found to improve health outcomes of clients, due to greater clinical collaboration and a focus on education.
